IDS Patient Policy, Procedure & Forms
Below are policies and procedures and standards followed by IDS as a Medicare participating Independent Diagnostic Testing Facility (IDTF). For additional questions or concerns, contact 800-355-0691, and ask for the compliance officer.
IDS Notice of Privacy Practices - Contains IDS Patient privacy policy.
Independent Diagnostic Testing Facility (IDTF) Standards - Federal standards to which IDS adheres.
Patient Compaint Procedure - This procedure outlines how IDS will handle patient complaints.
Assignment of Benefit / Medical Records Release - This is a blank copy of the form that is presented to you at the time of testing for signature. The Assignment of Benefit is to allow IDS to bill your insurance and receive payment for testing service. The Medical Records release is to authorize IDS to release test reports and other related items to treatment suppliers. You may request a copy of your original signed medical release and assignment of benefit form at any time by writing to the address below, or calling 800-355-0691 and verifying your identity.
Medical Release Revocation Form - Coming soon. Until the official form is published you may submit a written request to revoke a medical release. Include your full name, address, phone number, and date of birth and request that medical records no longer be shared with the specific party.
Medical Records Request Form - Official form coming soon, Allows you to request a copy of your medical records from IDS. Until form is published send a written request to the address below and include your full name, address, phone number, and date of birth and that you are requesting a copy of all medical records (or you may refer to a specific record).
IDS Hours of Operation:
IDS standard hours of operation are 8am through 4:30 pm Central Standard Time.
